State transitions are modelled by Element_notes added to an Element_issue.
Conditions: Goto "Accepted" by exactly one author. Goto "Fixed" by the same author as the last "Accept". Leave "Fixed" by a different author.
"SUM" is a pseudo-state, needed in EnumMaps for indexing the sum of all figures related to the particular states.
